Job Title: Social Worker - Home Health and Hospice
Job Location: Our Social Workers see patients in the most ideal setting - the patient's home!
Our office is located in Dunmore, PA
Shift(s): Full-time schedules available.
Office hours Mon - Fri 8:30 - 5:00.
Duties and Responsibilities include, but not limited to the following:

Assess patient care and plans with patient, family, and physician.

Assist the patient with ADL and facilitate their efforts towards self-sufficiency and independence.

Assess home environment regarding self-care, coping, and living arrangements.

Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (social services, area on aging, meals on wheels etc.) to carry out treatment plan objectives.
Requirements:
- Master's Degree in Social Work
- Current Pennsylvania LSW / LCSW preferred

Must have a current driver's license

Ability to travel to case assignments as assigned

Ability to work independently with minimal supervision and manage time effectively

Strong interpersonal skills
