Job Qualifications
Education:
Active and unencumbered Registered Nurse license.
Bachelor’s degree in nursing or related field required
Experience:
Minimum of five (5) years leadership and multi-state oversight experience required.
Minimum 5 years Home Health and Hospice experience.
Five (5) years proven experience in accreditation and survey process for hospice and home health.
Knowledge and Skills:
Detail oriented with strong organizational skills.
Demonstrates a strong ability to analyze problems.
Current knowledge of home health federal regulations and clinical practice
Joint Commission or similar accreditation experience preferred
Excellent communicator virtual, verbal and written
Excellent presentation skills virtual and live
Transportation: Reliable transportation and valid and current driver’s license and auto insurance
Travel: Travel Required
Environmental and Working Conditions:
Works in a routine office environment. Noise level may be moderately high. Ability to work a flexible schedule with extended hours. Ability to travel nationally with some exposure to inclement weather.
Physical and Mental Effort:
Prolonged sitting and some standing may be required. Occasional need to lift, pull, carry and push items weighing up to 50 lbs. Frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines and agency needs. Requires excellent problem-solving and team building skills.

Primary Responsibilities:
Leadership:
Develops inclusive strategic plans and strategies for quality program and agency accreditation by collaborating with the leadership team, clinical leaders and key stakeholders.
Communicate strategy effectively throughout the organization.
Lead and strategize with regional and senior leadership on the continuous evolution of the organizations customer driven culture that supports the delivery of excellence in patient care, outcomes and satisfaction.
Support organizational change and process improvement strategies by coaching and educating on tools and techniques (including, but not limited to Lean/Six Sigma techniques) for process improvement, and the need for the development of action plans used to implement and sustain changes necessary for improvement.
Patient Outcomes and Clinical Operational Excellence
Works with regional clinical directors, VP of Hospice and Home Health Clinical Operations and Chief Clinical and Quality officer to develop, evaluate, review, revise and implement policies and procedures.
Monitors key performance indicators and quality metrics for home health and hospice service line and collaborates with appropriate stakeholders on actions to meet defined benchmarks related to:
Outcome and Assessment Information Set (OASIS) Based Outcome Measures
Claims Based Outcome Measures
Home Health Care Consumer Assessment of Healthcare Providers and Systems (HH CAHPS) and Hospice CAHPS.
Program for Evaluating Payment Patterns Electric Report (PEPPER) for Home Health and Hospice
Comparative Billing Report (CBR)
Home Health and Hospice Compare – Star Rating
Quality Assessment and Performance Improvement (QAPI) Indicators
Utilizes data analytics to assess clinical performance and develop corrective action to achieve defined results.
Effectively monitors the high risk, high volume and problem prone data to ensure that the appropriate concurrent, proactive and retrospective audits are performed. Identifies trends. Ensures performance improvements plans are created and improvements are sustained through audit and education.
Team Leadership
Oversees a department that is responsible to organize, analyze, communicate and maintain performance improvement and maintain performance improvement data.
Provides support to regional clinical directors and operators to ensure regional performance meets defined outcomes
Provides direction to regional clinical directors related to patient care, agency clinical operational processes, policies and procedures, survey readiness and compliance with regulatory and accreditation requirements.
Provides support to regional clinical directors and agencies during survey preparation, self-assessments, on-site surveys and plans of correction.
Regulatory and Accreditation Oversight
Maintains current knowledge of current Medicare/Medicaid and The Joint Commission accreditation standards for home health and hospice.
Develops and ensures training and education for survey preparedness.
Ensures that QAPI Programs meet regulatory requirements, are monitored on an ongoing basis, evaluated for effectiveness and continuous improvement and that the agencies meet the priorities of care and safety.
Quality Assessment and Performance Improvement
Assures appropriate organization wide quality infrastructure and appropriate coordination of efforts across all service lines.
Ensures the organization meets all federal and state quality reporting requirements and meets goals to improve publicly reported quality measures, including but not limited to Hospice and Home Health star rating, Hospice Quality Reporting Program, Home Health Quality Reporting Program and PAC Quality Monitoring and Reporting Managers a process for all internal and external customer satisfaction surveys including survey development, data collection and analysis, and action planning for improvements that will meet the needs of all customers.
Support Performance Improvement Projects (PIP) through the proper utilization of staff and resources to identify and prioritize opportunities for improvement using data driven decision making.
Interact with key leadership to align QAPI activities with organization strategic goals.
