- Provide an interface between health care providers and Traditions Home Health Services.
- Develops and implements a program to contact all potential referral sources within the hospital.
- Responsible for establishing, growing, and maintaining referral volume and relationships with hospital staff.
- Communicates with existing referral sources and identifies new opportunities within the hospital
- Delivers exceptional customer service, and assists with advanced care planning discussions, and informative private home care consults for clients and family members
- Responsible for ensuring the intake and referral processes are performed effectively and timely to meet the needs of our clients, families and referral sources
- Completes the admissions process on referred clients in emergency cases, as staffing indicates, and for inpatient private home care admissions.
- Provides accurate and timely information regarding hospice services to anyone inquiringReceives calls from referral sources, responds timely, and initiates paperwork to begin Intake / referral process.
- Serves as client advocate in evaluating & developing plan of care.
- Offering in-services and education on the benefits of Private Home Care Services.
- Provides an interface between health care providers and Traditions Home Health Services.
