Position Overview:
The Community Health Worker (CHW) Manager will be responsible for overseeing and managing a team of Community Health Workers who support individuals with mental health challenges, helping them navigate the healthcare system and access the resources they need.
The CHW Manager will work closely with other departments to ensure that clients receive comprehensive and coordinated care while developing and implementing outreach strategies to engage with underserved communities.
Additionally, the CHW Manager will oversee the completion of assessments as needed to ensure that client needs are accurately identified and addressed. This position reports directly to the CEO.
Key Responsibilities:
Leadership & Supervision:
- Lead, supervise, and mentor a team of Community Health Workers, ensuring they are well-supported, well-trained, and motivated to provide high-quality care.
- Provide ongoing training and professional development opportunities for CHWs to enhance their skills and knowledge in behavioral health, cultural competence, and case management.
- Conduct regular one-on-one supervision sessions and team meetings to assess progress, address challenges, and ensure that team goals are met.
- Collaborate with the CEO to develop and refine the overall strategic vision for the CHW program.

- Program Management:
- Develop and oversee the implementation of community outreach programs aimed at increasing awareness and access to behavioral health services for underserved populations.
- Monitor and evaluate CHW program performance, ensuring that activities align with Tranquil Minds Behavioral Health's goals and objectives.
- Collaborate with clinical teams, case managers, and external partners to ensure integrated care and holistic support for clients.
- Ensure that assessments are completed as needed for all clients, including intake assessments, needs assessments, and follow-up evaluations.

- Client Support & Engagement:
- Oversee the assessment, planning, and coordination of services for individuals receiving care, ensuring they are connected with appropriate community resources and services.
- Ensure CHWs complete assessments as needed to evaluate clients' health, mental health, social needs, and barriers to care.
- Provide guidance to CHWs in addressing barriers to care, such as transportation, language, and cultural differences, and empower them to deliver compassionate, client-centered services.
- Ensure CHWs maintain confidentiality and adhere to ethical and legal standards of care, respecting clients' privacy and dignity.

- Data Collection & Reporting:
- Track and analyze data related to program outcomes, client engagement, and service delivery to assess program impact and effectiveness.
- Prepare reports and make recommendations for program improvements, ensuring that data is accurately reported in compliance with funding and regulatory requirements.
- Provide regular updates to the CEO on program performance, challenges, and opportunities for growth.

- Community Outreach & Collaboration:
- Cultivate relationships with local community organizations, healthcare providers, and public health initiatives to ensure that Tranquil Minds is a trusted partner in mental health care.
- Develop and implement strategies to engage with community members, educate them about available behavioral health services, and reduce stigma surrounding mental health.
- Represent Tranquil Minds at community events and partnerships, ensuring the organization’s mission and services are well-represented.
Qualifications:
- Bachelor's degree in Public Health, Social Work, Psychology, or a related field (required). A Master's degree is a plus.
- At least 3-5 years of experience in community health work, mental health services, or a related field.
- Previous experience in a supervisory or management role.
- In-depth understanding of behavioral health services, social determinants of health, and the challenges faced by underserved communities.
- Strong interpersonal skills, with the ability to connect with clients from diverse backgrounds and build trusting relationships.
- Knowledge of community resources and the ability to navigate the social services landscape.
- Excellent communication skills, both written and verbal.
- Ability to work collaboratively with a multidisciplinary team.
- Proficiency in Microsoft Office Suite and experience with data collection and reporting systems.
