What are the responsibilities and job description for the Community Health Worker position at Tranquil Minds Behavioral Health?
Responsibilities:
1. The CHW will provide psychosocial rehabilitation training that assists patients with life skills, coping skills, independent living skills, housing and home maintenance, personal management skills, vocational skills, employment skills, and education on medication management. Skills training will occur individually and/or in a group format if needed.
2. The CHW will provide case management services to assist patients with accessing community resources. The CHW will develop working relationships with community agencies and work collaboratively with internal and external providers and care team members. The CHW will maintain flexibility to deal with clinical needs. The CHW will document all attempts to provide services and no-shows. The CHW will ensure all notes are signed with credentials, dated, and turned in within policy requirements. The CHW will ensure continued patient and community customer satisfaction.
3. The services provided are intended to help patients develop a natural support system so that they may live as independently as possible within the community. The specific needs of the patient will be identified through an assessment process and a treatment plan will be developed, in conjunction with the patient, to determine goals, objectives, and interventions.
Qualifications:
-The job requires a bachelor's degree in psychology, social work, human services or a related field
-Applicant must possess and maintain a current/ valid state Driver's License with an acceptable driving record
-Certification as a Qualified Mental Health Professional (QMHP) is required
- Knowledge of medical terminology and healthcare systems
- Strong communication skills, including public speaking and the ability to educate others effectively
- Experience in data collection
- Familiarity with motivational interviewing techniques is a plus
- Ability to work independently and as part of a team
- Compassionate, empathetic, and culturally sensitive approach to working with diverse populations
