Description: Hospice LPN - Provide Hospice Nursing services offering emotional, social and spiritual care and support to patients and their care givers.
Scope of Work:
- Nursing: Assist the RN/Case Manager with providing hospice care in accordance with the patient's plan of care. Assist the RN/Case Manager in performing specialized procedures as directed by attending physician and Hospice Medical Director.
- Documentation: Submit complete, accurate and relevant clinical notes in a timely manner. Assist the RN/Case Manager with the preparation, implementation, and continuing evaluation of the patient plan of care and inform the RN/Case manager of changes noted during visits.
- Communication: Assist in providing and reinforcing education to patients/family members regarding the dying process Obtain a medical history from the patient and/or a family member particularly as it relates to the present condition.
- Technology: Use of Electronic Health Record, Email, computers, and cell phone devices.
Qualifications:
- Must be a graduate from an accredited School of Nursing.
- Must be licensed in the State of Ohio as a Licensed Practical Nurse.
- Have a valid, unencumbered LPN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state.
- Have one year of LPN experience. Hospice experience preferred.
- Have excellent assessment skills and problem-solving skills.
- Be skillful in organization and in the principles of time management and have knowledge of symptom management.
- Be a licensed driver with a reliable, insured automobile.
- Must be able to communicate well, both verbally and in writing
Education:
- Must be a graduate from an accredited School of Nursing.
- State of Ohio License - Registered Nurse (Unencumbered)
Job Conditions:
- Physical activities will include, walking, sitting, stooping, and standing.
- Travel 100%
- Able to lift fifty pounds.
