Job Summary
We are seeking a compassionate and dedicated LVN Field Nurse on the weekends to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including their homes and community facilities. This role requires a strong understanding of patient needs, effective communication skills, and the ability to work independently while adhering to established healthcare protocols.
Responsibilities
- Deliver comprehensive nursing care to patients in their homes or community settings.
- Conduct assessments and develop individualized care plans based on patient needs.
- Administer medications and treatments as prescribed by healthcare providers.
- Educate patients and their families on health management, including family planning and dementia care.
- Maintain accurate patient records in compliance with HIPAA regulations using electronic health record systems such as Epic.
- Collaborate with multidisciplinary teams to coordinate patient care and case management.
- Provide support for inpatient services when necessary, including catheterization and other medical procedures.
- Stay updated on best practices in nursing and participate in ongoing professional development.
Skills
- Strong knowledge of physiology and nursing principles.
- Experience in family planning and dementia care is highly desirable.
- Proficient in case management practices and patient education techniques.
- Familiarity with electronic health record systems, particularly Epic.
- Ability to work effectively both independently and as part of a team.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Commitment to maintaining patient confidentiality and adhering to HIPAA guidelines.

Qualifications
- Current LVN license
- Proficiency in catheterization and medication administration

Pay: $35.00 - $40.00 per hour
