LVN Per Diem

Job Summary
We are seeking a compassionate and dedicated LVN Field Nurse on the weekends to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including their homes and community facilities. This role requires a strong understanding of patient needs, effective communication skills, and the ability to work independently while adhering to established healthcare protocols.

Responsibilities

• Deliver comprehensive nursing care to patients in their homes or community settings.
• Conduct assessments and develop individualized care plans based on patient needs.
• Administer medications and treatments as prescribed by healthcare providers.
• Educate patients and their families on health management, including family planning and dementia care.
• Maintain accurate patient records in compliance with HIPAA regulations using electronic health record systems such as Epic.
• Collaborate with multidisciplinary teams to coordinate patient care and case management.
• Provide support for inpatient services when necessary, including catheterization and other medical procedures.
• Stay updated on best practices in nursing and participate in ongoing professional development.

Skills

• Strong knowledge of physiology and nursing principles.
• Experience in family planning and dementia care is highly desirable.
• Proficient in case management practices and patient education techniques.
• Familiarity with electronic health record systems, particularly Epic.
• Ability to work effectively both independently and as part of a team.
• Excellent communication skills for interacting with patients, families, and healthcare professionals.
• Commitment to maintaining patient confidentiality and adhering to HIPAA guidelines.

Qualifications

• Current LVN license
• Proficiency in catheterization and medication administration
• Experience in patient care and dementia care
• Knowledge of EHR systems and care planning
• Strong communication skills and ability to work in a team
• Familiarity with case management and family planning practices
• Preferably Bilingual

Pay: $35.00 - $40.00 per hour
