JOB SUMMARY
Transcend Hospice & Palliative Care is seeking an RN On Call who will be responsible for ensuring that patient care is coordinated and managed appropriately. A typical schedule will be from Monday 5:00 pm to the following Monday 8:30 am, with 7 days on and 7 days off. The On-Call RN is responsible for ensuring that care and services are delivered per Transcend Hospice & Palliative Care policy and Medicare guidelines. The On-Call RN will triage calls and provide on-call visits, pronouncements, and admissions as needed during scheduled on-call hours throughout our Burlington, Camden, and Gloucester Counties service areas.
JOB RESPONSIBILITIES:
- Making home visits to clients in designated geographic territories.
- Performing assigned duties, including administration of medication, wound care, treatments, procedures, and comfort care.
- Monitoring clients' conditions and reporting changes to the Clinical Manager.
- Following up with, executing, and properly documenting doctors' orders.
- Performing client assessments as necessary.
- On-call after-hours and weekend visits.
- Occasional after-hours admissions.
- Ensure that documentation is done promptly in the EMR.
QUALIFICATIONS/SKILLS
- A current license as a Registered Nurse (RN) in NJ.
- At least two years of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience is strongly preferred but not required.
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
- Must have a valid driver’s license and auto insurance.
Work Schedule: Rotating schedule—on-call from Monday at 5:00 pm to Monday at 8:30 am—7 days on and then 7 days off!
Competitive Compensation: Flat pay per day plus additional incentives for admissions, on-call visits and mileage reimbursement.
