Transcend Hospice & Palliative Care is seeking a compassionate and dedicated Hospice RN Case Manager to join our team. In this role, you will provide high-quality, patient-centered hospice care, ensuring comfort, dignity, and support for patients and their families. As a key member of the interdisciplinary team, you will coordinate and oversee individualized care plans, symptom management, and end-of-life support in home or facility settings.This role requires a Hospice RN Case Manager to travel to our service areas of Burlington, Camden & Gloucester counties.
Job Responsibilities:
- Provide skilled nursing care to hospice patients in accordance with their care plans.
- Assess patient conditions, manage symptoms, and adjust care plans as needed.
- Educate and support patients, families, and caregivers on end-of-life care and comfort measures.
- Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic patient care.
- Document patient status, treatments, and interventions in compliance with hospice regulations.
- Participate in interdisciplinary team meetings and contribute to care planning.
- Maintain compliance with Medicare, Medicaid, and state/federal hospice regulations.
- Participate in on-call rotations as needed.
Qualifications/Skills:
- Education: Associate or Bachelor’s Degree in Nursing (ASN/BSN) from an accredited program.
- Licensure: Active and unrestricted Registered Nurse (RN) license in New Jersey.
- Experience: Minimum of two year of nursing experience, preferably in hospice, palliative care, or home health.
- Certifications: CHPN (Certified Hospice and Palliative Nurse) preferred but not required.
- Skills: Strong clinical assessment, communication, and organizational skills.
- Other: Must have a valid driver’s license and reliable transportation for patient visits.
