Job Description

We are seeking a Charge LPN to join our team and lead our Dementia Unit! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Position Summary

Oversees the Seasons Recreation Program in accordance with policy and procedures. This position is responsible

for the overall Alzheimer’s and Dementia training and programming, sales/marketing, and supports overall

occupancy. This position also develops, coordinates and implements programs for patients with cognitive

impairment in a therapeutic environment that maintains each patient’ highest level of physical, social and

psychological well-being.

Conducts job responsibilities in accordance with the standards set forth in Transcendent Healthcare’s Code of

Business Conduct, Transcendent Healthcare’s policies and procedures, applicable federal and state laws, and

applicable professional standards.

Provides nursing services within the standard responsibilities of a Licensed Nurse. This is a universal position

providing all aspects of nursing services, recreational services and social services.

Essential Functions: Direct involvement and oversight of Recreation Coordination and participation of the

following:

– Is responsible for meeting occupancy expectation and census growth

– Is responsible for SCU marketing, community education, and community relations.

– Must be able to work cooperatively as a member of a team.

– Keeps confidential all patient information received.

– Responsible collectively with the DNS and Nursing staff for documentation of patient condition;

records in the clinical record and reports all observed symptoms, reactions, treatments and changes

in the patient’s condition.

– Assesses and documents the patient’s condition and nursing needs. Sets goals and prescribes

appropriate nursing intervention in conjunction with the DNS and Nursing staff to meet the physical,

psychological, social rehabilitative and discharge planning.

– Notifies nursing staff immediately, as needed, related to condition change of resident.

– Administers chart medications as ordered by physician.

– Assists physician with examination, treatments, special tests and procedures a defined within the

scope of practice.

– Obtains, carries out and follows physician orders.

– Assists with rehabilitation of patients in accordance with the medical care plan.

– Participates in patient care for Seasons patients.

– Interviews new unit personnel and recommends hires and terminations. Participates in counseling,

disciplinary action and termination of employees.

– Prepares written job performance evaluations and conducts employee performance conferences.

– Trains and orientates new and existing personnel on an ongoing basis. Responsible for assuring that

required dementia training occurs for all new hires and existing personnel.

– Manages Seasons unit which includes planning daily and/or weekly work assignments and breaks.

Ensures adequate staffing and directs duties of all Seasons staff.

– Assists unit personnel, as a universal position, in interpreting and planning assignments and in

executing difficult assignments.

– Exercises independent judgment and decision making.

– Establishes and maintains communications and utilizes teaching opportunities with patients, and

other significant individuals.

– Communicates effectively with patients and their families, physicians and staff.

– Participates in team conferences, Unit Manager daily reviews, development of nursing care plans, and

team and shift reports.

– Actively participates in Quality Assurance Committees.

– Answers call lights, assists patients with personal hygiene and other activities of daily living as a

universal position.

– Assists in assessing, admitting, discharging and transferring patients.

– Complies and performs within the scope of practice a defined by state Nursing Practice Act.

– Completes all documentation and compliance of local, state and federal guidelines and survey

processes.

– Develop activity calendar and lead activities programs as is appropriate to provide optimal

recreational therapy to support stimulating, but not over stimulating, prolonging the seven stages of

the diagnosis.

– Maintains necessary records/documentation of activity programs including activities attendance and

care plan.

– Design activities programs that meet the physical, social, emotional, and intellectual needs of the

residents.

– Identify activities which will meet residents’ needs and adapts them to the resident’s interests, talents

and abilities.

– Evaluates effectiveness of programs in terms of resident outcomes. Evaluate if resident is no longer

benefiting from the SCU’s programming and recommend relation to the DNS.

– Advises nursing, dietary, maintenance, volunteers, etc. of related responsibilities for an activity in

advance if necessary.

– Helps to identify facility and community resources that can provide identified activities.

– May coordinate Seasons newsletter or other promotion of activities programs.

– Promotes a volunteer activities staff/program which utilizes community involvement as appropriate.

– Secures entertainment, program leaders, volunteers, materials, equipment, transportation (as

necessary) well in advance of scheduled dates.

– Assures all residents are involved in activities; group, individual and/or independent on a weekly

basis.
