Job Description
THIS POSITION IS FOR A DEMENTIA UNIT!
Position Summary
Overseas the Seasons Recreation Program in accordance with policy and procedures while working in the dual
role, dual capacity as a certified nurse's assistant and activities coordinator. Develops, coordinates and implements programs for patients
with cognitive impairment in a therapeutic environment that maintains each patient’s highest level of physical,
social and psychological well-being.
Conducts job responsibilities in accordance with the standards set forth in Transcendent Healthcare’s Code of
Business Conduct, Transcendent Healthcare’s policies and procedures, applicable federal and state laws, and
applicable professional standards.
Provides nursing services within the standard responsibilities of a Certified Nursing Assistant. This is a universal
position.
Essential functions:
– Develop activity calendar and lead activities programs as is appropriate.
– Maintains necessary records/documentation of activity programs including activities attendance and
care plan.
– Design activities programs that meet the physical, social, emotional, and intellectual needs of the
residents.
– Identify activities which will meet residents’ needs and adapts them to the resident’s interests, talents
and abilities.
– Evaluates effectiveness of programs in terms of resident outcomes.
– Advises nursing, dietary, maintenance, volunteers, etc. of related responsibilities for an activity in
advance if necessary.
– Helps to identify facility and community resources that can provide identified activities.
– May coordinate Seasons newsletter or other promotion of activities programs.
– Promotes a volunteer activities staff/program which utilizes community involvement as appropriate.
– Secures entertainment, program leaders, volunteers, materials, equipment, transportation (as
necessary) well in advance of scheduled dates.
– Assures all residents are involved in activities; group, individual and/or independent on a weekly
basis.
– Mail delivery to residents.
– Coordinates physical exercise program, mental stimulation for residents.
– Maintains awareness of therapeutic diets of individual residents.
– Assists with serving meal trays, snacks.
– Monitors residents closely during activities and does everything possible to maintain resident safety.
– Notifies nursing staff immediately, as needed, related to condition change of resident.
– Maintains compliance of monthly budget for department by utilization of purchase order system.
– Provides and/or assists patient with activities of daily living such as grooming; oral hygiene;
bath/shower, hair; feeding and nourishment and incontinence care.
– Maintains clean and safe environment by insuring each patient unit is well organized.
– Performs and/or assists patients with passive range of motions exercises; ambulation, transfers
to/from bed, floor.
– Responds to call lights, patients and visitors’ requests and corrects environmental hazards in an
appropriate and safe manner.
– Provides accurate, timely documentation as required within the scope of practice.
– Obtains/performs vital signs, heights, weights, intake and output measurements, etc and documents
meal/nourishment consumption.
– Attends in-service training and is responsible to maintain required number of in-service hours and
current state certification along with SCU established requirements.
– Adheres to standard precautions in daily work.
– Follows and carries out directions of Program Director and other managerial staff.
– Observes facility policies and procedures regarding attendance, timelines, house rules, teamwork,
customer service and other policies/procedures that may be introduced from time to time.
– Must be able to work cooperatively as a member of a team.
– Keeps confidential all patient information received.
– Performs other duties as assigned.
