Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will have experience working with individuals with developmental disabilities and possess a strong background in both inpatient and clinic settings. This role involves providing high-quality patient care, conducting assessments, and collaborating with healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and perform IV insertions as needed.
- Collaborate with multidisciplinary teams to facilitate discharge planning for patients.
- Provide urgent care services, addressing acute health issues promptly.
- Utilize Epic software for patient documentation and management.
- Support patients with developmental disabilities through tailored care approaches.
- Perform sonography as required for diagnostic purposes.
- Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
- Educate patients and their families about health management and preventive care strategies.
Skills
- Strong understanding of working with individuals with developmental disabilities.
- Proficiency in using Epic electronic health record systems.
- Experience in inpatient care settings as well as outpatient clinic environments.
- Competence in medication administration and IV insertion techniques.
- Knowledge of discharge planning processes to ensure smooth transitions for patients.
- Ability to lift patients safely and assist with mobility as needed.
- Familiarity with sonography procedures is a plus.
- Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare teams.
- Strong problem-solving abilities and a commitment to providing compassionate patient care.
