Transformation House is licensed by the Minnesota Department of Human Services, we are a 245G Substance Use Disorder Treatment Facility. We have three residential sites, in Anoka and Fridley, and we have two sober living facilities, in Champlin and Fridley.

Description
Transformation House is hiring! Are you a Certified Nurse Practitioner or Psychiatric Nurse Practitioner that is looking to work with a great team? Look no further. The perfect candidate would have experience working in the substance use disorder / mental health field, provide expert client care, and documentation.
This position would be responsible for :
- Providing Medication Management (including suboxone)
- DA's (if applicable)
- Making Referrals
- Consulting with the Multidisciplinary Team
- Provide accurate and timeley documentation
Transformation House has three residential sites, and an expanding outpatient program at our Recovery Hub. This position would provide care to both residential and non-residential clients.
