Transformation House is licensed by the Minnesota Department of Human Services, we are a 245G Substance Use Disorder Treatment Facility. We have three residential sites, in Anoka and Fridley, and we have two sober living facilities, in Champlin and Fridley.

Job Responsibilities (not meant to be all-inclusive)
- Comprehensive Assessments
- Case Management
- Skilled in performing the 12 core functions
- Knowledge of Minnesota Matrix Six Dimension
- Required Documentation, completed on time per licensing requirements.
- Facilitate therapeutic group sessions
- Provide Individual Sessions
