We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality care to our outpatient clients at Recovery Hub.
Position Overview
As an LPN, you will play a crucial role in supporting our clients’ well-being by delivering essential nursing services, ensuring compliance with health regulations, and collaborating with a dedicated team of providers.
Key Responsibilities
- Deliver professional nursing care and education to clients in an outpatient setting.
- Monitor, document, and report any changes in client conditions.
- Transcribe and manage physician orders in client charts.
- Maintain accurate and thorough nursing notes reflecting care and client responses.
- Participate in shift change reports and weekly staffing meetings.
- Work closely with in-house healthcare providers to support client care plans.
- Assist with prior authorizations for client medications.
- Conduct scheduled chart audits to ensure compliance.
- Adhere to Minnesota Department of Health regulations.

Requirements
- Have an active Licensed Practical Nurse (LPN) license through the Minnesota Board of Nursing
- Ability to pass a DHS background study
- CPR-First Aid Certified
