We are located in Houston, TX (Harris County). Ultimately, we will serve the vast majority of Harris County initially, our service will start in the East and Southeast Region of Houston at initial phase of service. (Baytown, LaPorte, Clear Lake, etc.)
We are excited to be able to contribute to this grow in this amazing healthcare direct-community with our newly formed company, contributing to all patients with a continuum of care and long-term care services. It is our goal to DELIVER excellent and patient experiences to every patient and resident family member.
CNA/ Hospice Health Aide - PRN
Service Areas : Clear Lake & La Porte, Baytown
Responsibilities:
- Provide exceptional care in coordination with the assigned nursing care team.
- Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
- Help with patient meals, feeding, and basic activities of daily living.
- Deliver weekly reports to the case manager and review any important changes in patient conditions.
- Educate patients and family members.
- Prepare timely, complete, and accurate clinical notes.
- Communicate effectively with healthcare providers and other care team members.
Qualifications:
- CNA license in the state of Texas (required).
- One (1) year hospice experience.
- Able to lift up to fifty (50) pounds.
- Able to work occasional weekends, holidays, evenings.
- Strong communication and people skills.
- Experience with HCHB EMR (preferred but not required)
