Licensed Vocational Nurse or Licensed Practical Nurse
(LVN or LPN) - Home Health
Shifts Available: Full-Time & PRN
Service Areas with Openings: Northeast / Northwest / Southeast / Woodlands / Clear Lake
Responsibilities:
- Collaborate with the home healthcare team to develop and implement individualized care plans.
- Complete home care nursing visits.
- Prepare timely, complete, and accurate clinical notes.
- Communicate with healthcare providers and other care team members.
- Educate patients and their families about health conditions, treatment plans, and self-care techniques.
- Adhere to all agency policies, procedures, and regulatory requirements related to home health care.
Qualifications:
- Valid and current LVN or LPN license in the state of Texas or compact state.
- Minimum of one (1) year of experience in a home health setting.
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR. (preferred but not required)
- Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
- Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
