We are located in Houston, TX (Harris County). Ultimately, we will serve the vast majority of Harris County initially, our service will start in the East and Southeast Region of Houston at initial phase of service. (Baytown, LaPorte, Clear Lake, etc.)

The Job
The Hospice Patient Care Coordinator plays a key role in coordinating all aspects of the patient admission and care planning process. This includes managing intake calls, processing referrals, overseeing medical records, scheduling field staff, and collaborating with the clinical team to ensure smooth, timely admissions. The ideal candidate must have prior experience using Homecare Homebase and be comfortable working in a growth environment.

Primary responsibilities will be:
- Receive and process all patient referrals and intake inquiries
- Coordinate and verify insurance, physician orders, and required documentation
- Maintain and manage up-to-date patient medical records and files in compliance with regulatory standards
- Serve as the primary admissions coordinator, facilitating timely evaluations and initiations of care
- Schedule field staff visits in alignment with patient needs and staffing availability
- Collaborate closely with the Administrator, and interdisciplinary team to ensure patient care is well-coordinated and efficient
- Serve as Volunteer coordinator and liaison between, facilities, patients, families, and the hospice team
- Monitor and follow up on pending admissions or incomplete documentation
- Utilize Homecare Homebase (HCHB) to track and manage patient information, and use of forcura for monitoring faxes and referrals
- Providing an outstanding admissions experience for our patients
- Communicating effectively with patients, family members, Case Managers, healthcare providers and co-workers
- Reviewing patient charts and ensuring information is clearly communicated to the patients care team
- Serving as a point of contact throughout the admissions process
- Ensuring coordination of care with other team members
Must-have’s to be considered for this position:
- Minimum 2 years of experience in hospice or home health coordination required
- Proficient in Homecare Homebase (HCHB) – required
- Strong knowledge of hospice regulations, documentation standards, and Medicare guidelines
- Excellent communication and customer service skills
- High level of accuracy and attention to detail
- Able to multitask and prioritize in a high-volume environment
- Must be organized, proactive, and solution-focused
- CNA certification a plus but not required
- High School Diploma or equivalent required
