TSINY, a mental health services agency is seeking Activities Coordinator for its Residential Mental Health program located in Queens Village, New York.

Local travel between Residential Programs may be required.

Salary : $47,320; $26 Per Hour, Full- Time

Driver License Required

Position Summary

Coordinate recreational activities for the residents. Maintain a relationship with HAI to obtain tickets to offsite events.

Organize special events, such as barbeques and Family and Friends Day event.

Essential Functions

  • Read Communication Book and initial.
  • Converse with shift going off duty for any pertinent information, i.e., appointments, overall resident’s behaviors, changes in medication, etc.
  • Dispense and / or supervise morning, afternoon and evening medication. Initial medication book immediately after each individual medication is given to a resident.
  • Follow procedure in accordance to change of shift checklist and sign.
  • Complete a room check to determine the number of residents in the facility and status of each resident.
  • Fill out resident daily sheet, if assigned.
  • If taking a call-in (before supervisor arrives), record staff absence and / or lateness in Call-in Book.
  • Alert day programs of resident's absence and lateness if resident has an appointment.
  • Check staff revised schedules.
  • Check personal mailbox located in the counselors' office.
  • Attend shift team meeting (morning or evening) with shift supervisor.
  • Follow daily schedule as posted or assigned.
  • Assist residents with meal preparation when needed.
  • Report any pertinent information regarding residents and facility to shift supervisor throughout your shift.
  • If assigned, put medication up in medication bins and check.
  • Document in Communication Book any pertinent information regarding residents.
  • Conduct individual and group activities.
  • Check with supervisor of shift for additional assignment after conducting groups.
  • Supervise residents during free time.
  • Review, evaluate and assess residents’ participation in groups.
  • Educate and encourage residents to use leisure time appropriately and constructively.
  • Conduct cultural diversity groups.
  • Follow procedure in accordance to change of shift check list with on-coming shift and sign change of shift checklist where needed.
  • Plan special events for weekends or evenings.
  • Coordinate special events with shift supervisor and direct supervisor.
  • Check revised staff schedule before assigning staff to escort residents on off-site activities.
  • Meet with Activities Counselor for supervision.
  • Meet with Director for supervision.
  • Escort residents to appointments if assigned (check appointment book or calendar).
  • Escort residents shopping for personal needs.
  • Write Medicaid Progress Notes reflecting restorative services, if applicable.
  • Attend weekly staff meeting. Read staff meeting minutes if absent.
  • Coordinate Special Olympic activities.
  • Post weekly activities / groups and excursions.
  • Submit monthly statistics.
  • Submit travel vouchers.
  • Review residents' services plans.
  • Submit supplies and materials needed to conduct groups as needed.
  • Attend service plan meetings on assigned residents.
  • Participate and supervise residents during fire drills.
  • Complete monthly activities report in all residents' charts.
  • Submit board notes report.
  • Evaluate residents' ADL skills (assigned by case manager).
  • Review with resident’s service plan goals, if assigned.
  • Update bulletin boards / decorate and coordinate activities for specific parties / holidays and birthdays.
  • Functional assessment completed, if assigned.
  • Check with case manager for annual forms to be completed or updated in charts.
  • CPR / First Aid training
  • Coordination of annual "Family and Friends Day" event.
  • Coordinate resident holiday activities and parties.
  • Make sure Special Olympics medical forms are completed and returned.
  • Make appointment to meet with Director for annual employee evaluation.
  • Complete annual employee evaluation for activities counselor.
  • Submit list of supplies and materials needed to conduct groups and activities.
  • Teach residents socialization skills.
  • Incident reports completed on same day of incident and report given to shift supervisor or administrators.
  • Submit receipts for petty cash spent.
  • Arrange resident meetings with supervisor for special problems, clarification or assessment.
  • Shop for miscellaneous program or group needs.
  • Inform residents of appointments.
  • Attend Agency-wide meetings.
  • Attend special on-site and off-site training, seminars, conferences, etc.
  • Transport / escort residents to appointments or extra curriculum and recreational activities.
  • Availability to residents on an as needed and crisis basis.
  • Submit request for time off, at least two weeks in advance.
  • Check revised schedules for possible changes in hours / shifts and for holiday coverage.
  • Visit and document visits to hospitalized residents on hospitalization form in resident chart.
  • Document all travel training.
  • Submit new ideas for groups.
  • Return correspondence that states, "Sign and return."
  • Familiarization with personnel policy and emergency manuals.
  • Submit time sheets bi-weekly.
  • Provide supervision for residents during dining hours, if assigned.
  • Make sure residents are dressed appropriately before leaving the facility.
  • See attached "Chart Documentation Update Guide."
  • On committee(s) for Agency-wide special events.
  • Be able to sit or stand as needed, with or without reasonable accommodation.
  • May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation.
  • Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.
  • Driver’s license required.
  • During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
  • Perform other related duties as required.

Experience / Education / Skills / Abilities

High School diploma plus 1-years’ experience in planning recreational and social events for special needs populations preferred.
