Description
Provide on-site support and supervision in a group home setting specifically designed for seniors (55 ), with an emphasis on maintaining a high quality of life through activities, services and community engagement. Instruction and assistance with budgeting, community utilization, socialization, use of leisure time, and activities of daily life. Writing quality progress notes that are accurate, clear, comprehensive, contemporaneous and unique to each rehabilitation intervention provided. Meeting with residents on a regular basis to evaluate progress and develop new objectives and activities as needed.
Qualifications
Must possess 1 of 2
- Master's degree in Social Work, Mental Health Counseling, Counselor Education, Rehabilitation Counseling or related field
- Bachelor's degree in Psychology, Social Work, or related field; and one year paid direct service experience in a mental health or human services setting
Must possess a valid driver's license and reliable personal automobile, pass all background and drug clearances.
