EXEMPT/NON-EXEMPT: Non-Exempt
SUPERVISOR'S TITLE: Clinical director
SUMMARY:
Under the supervision of the Clinical Director, the Team Leader provides clinical and administrative support to the assigned team members and the beneficiary /family. This position is crucial in the provision of services and collection of information to allow for determination of beneficiary progress, continued stay criteria, and/or transition in services. The Team Leader is responsible for multiple supervisory tasks related to the functions of the services, including coordinating schedules, providing direct services, maintaining clinical records, and the managing multiple services requirements as identified in Service Definition.
DUTIES AND RESPONSIBILITIES:
- Provide the clinical and administrative supervision of all members of the team for the provision of the services.
- Providing weekly meetings with team members
- Coordinating the assessment and reassessment of the recipient's clinical needs as required by the service definition.
- With help from the Clinical Director, designate the appropriate team member for everyone being served.
- Consult with identified providers, engaging community and natural supports as requested, and including their input in the PCP process.
- Monitor and evaluate the effectiveness of the interventions, as evidenced by symptom reduction and progress toward goals identified in the PCP .
- Ability to work with multi-cultural/ multi-lingual populations and multi
disciplinary teams.
- Coordinates therapeutic activities/schedules for team members.
- Ensures the appropriate use of authorized units within the authorization period by managing team’s units and authorizations.
- Exercises discretion and adheres to HIPAA and confidentiality standards and client’s rights when working with management and direct service staff within and outside Transitional therapeutic services, LLC facilities.
- With support from the Clinical Director, oversees the intake and assessment process including review of PCP and updates, MCO Documentation, weekly progress notes, monthly PCP updates, etc. This includes gathering and reviewing
relevant information to determine appropriateness of continued stay in service. Assess information regarding services along the continuum of care that may the needs of the beneficiary .
- Attends clinical supervision with the clinical director.
- Protects the confidentiality of sensitive information including verbal and written communications related to clinical information.
- Obtains and increases professional and clinical knowledge and skills by attending trainings, reviewing professional publications, and establishing professional networks.
- Delivers reports to the management team in the areas of incident reports, staffing issues, and other reports as directed by the Clinical Director and QA/QI according to organizational policy and procedure requirements.
- Completes tasks with attention to detail, meets established deadlines, and functions with minimal direct supervision.
- Demonstrates the ability to organize and prioritize required and assigned job duties.
- Team lead lays functional interpersonal skills with employees and beneficiary s by communicating in a clear and direct manner when relaying information and managing tasks.
- Maintain positive and professional attitudes and communications while interacting with clients, families, stakeholders, and colleagues.
- Demonstrates the ability to supervise the provision of services to clients in the office as well as in the community.
- Demonstrates flexibility as it relates to a change of work hours on short notice.
- Performs other duties and special projects as assigned by the CEO, Clinical Director, or QA/QI compliance officer.
- Participation in the initial development and ongoing revision of the PCP and ensuring its implementation.
- Monitoring and documenting the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the PCP.
WORK SCHEDULE: A Flexible schedule is required to accommodate beneficiary s, families and stakeholders which may include evening, weekend hours etc....
STAFF REQUIREMENTS:
- Must be either a licensed or board-eligible/provisional licensed professional as a:
- Licensed Clinical Social Worker (LCSW or LCSWA)
- Licensed Clinical Mental Health (LCMHC or LCMHCA),
- Licensed Psychological Associate (LPA),
- Licensed Marriage and Family Therapist (LMFT or LMFTA),
- Must have a minimum of one (1) year documented experience with the population
- Must have a valid, unrestricted, state appropriate driver’s license
- Must possess the visual acuity to develop written correspondence and determine accuracy, neatness, and thoroughness of the work assigned.
- Must be able to physically perform the basic life operational functions of talking, hearing, sitting for long periods, and performing repetitive motions, such as using a computer.
- Must be able to speak audibly, write legibly and perform repetitive motions such as typing.
- Must be able to perform light/moderate lifting and carrying, for items such as marketing materials and marketing displays.
KNOWLEDGE, SKILLS, AND ABILITIES:
- The Team Leader will possess a working knowledge of the mental health field
- Must be able to use appropriate judgment when serving individuals.
- Team Leaders will be able to maintain current records in an organized system.
- Computer and database skills are required for the Team Leader position.
- Strong interpersonal skills and the ability to effectively communicate with a wide range of individuals within a diverse community.
- Ability to effectively supervise and manage a team.
- Ability to apply therapeutic techniques. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.
- Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills.
- Ability to handle multiple tasks and meet critical deadlines. Ability to meet short and long-term goals and project deadlines.

Pay: $40.00 - $50.00 per hour
