What does a Transitions at Home Intake Coordinator do?
Transitions At Home Intake Coordinator serves as the first point of contact for discharge planners, physicians, and agency staff, processes all referrals for home care services, coordinates admissions, maintains patients files. Works under the direction and supervision of the RN Clinical Manager and Administrator.

Works under the direction and supervision of the RN Clinical Manager and Administrator.
Hours:
- 40 hours per week 8:00 am - 4:30pm
- Monday through Friday
- No Night Shifts (also known as PM and NOC Shifts)
- No Holidays

Qualifications / Job Requirements:
- High school diploma or equivalent
- One year experience in Home Health (preferred)
- Excellent written and verbal communication skills
- Private Insurance Billing
- Manage Medical records/EMR
