What does a Transitions at Home NM Lead Caregiver do?
The Non-Medical Lead Caregiver is responsible for Caregiver introductions, training the Caregivers, matching caregivers with Clients, VA Authorization renewals, and performing services for the Client as necessary to maintain the personal care and comfort of the Client, while in their own home.

What does a Transitions at Home NM Lead Caregiver do?
The Non-Medical Lead Caregiver is responsible for Caregiver introductions, training the Caregivers, matching caregivers with Clients, VA Authorization renewals, and performing services for the Client as necessary to maintain the personal care and comfort of the Client, while in their own home.
Hours: Flexible
- 8:00am - 4:30pm
- Monday through Friday
- No Holidays

Qualifications / Job Requirements:
- High School Diploma or GED Certificate
- Previous experience in Home Health Care
- A valid Wisconsin Driver's License and Car Insurance
- Reliable Car
- Basic Computer Skills
- Knowledge of medical terminology
- Excellent communication skills with attention to detail
- Must be able to read, write and communicate in English
