We are seeking a full-time RN Case Manager to join our team at Transitions At Home. As a caring and compassionate RN, you will provide skilled nursing care to patients in their place of residence, including assessments, medications, treatments, and coordination of services.
Job Responsibilities
- Provide skilled nursing care, including assessments, medications, treatments, and coordination of services.
- Counsel patients and family members/significant others on healthcare plans and goals.
- Coordinate with healthcare providers to ensure quality patient care.

Qualifications/Requirements
- Active Wisconsin Registered Nurse License.
- Two years' nursing experience, one year in Home Health (preferred).
- CPR Certification.
- Valid Wisconsin Driver's License and Car Insurance.
