What does a Transitions at Home Occupational Therapist do?
An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

What does a Transitions at Home Occupational Therapist do?

Hours: flexible
- 8am -430pm
- Monday through Friday
- No Holidays
- $40- $45 per hour starting wage, depending on experience

Qualifications / Job Requirements:
- Active Wisconsin Occupational Therapist License
- Two (2) years’ experience preferred.
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
