As a Hospice Hospital and Post-Acute Liaison at Transitions Care you will be responsible for establishing and maintaining relationships with hospitals and post-acute care settings (e.g., nursing homes, rehabilitation centers, assisted living facilities) to promote and drive referrals to our hospice services. You will serve as a key point of contact between our hospice organization and healthcare providers in both hospital and post-acute settings to increase awareness, educate on our services, and ensure a steady stream of patient admissions. The ideal candidate will have a strong background in sales, excellent communication skills, and a passion for providing compassionate end-of-life care. Represent Transitions and its services in a competent professional and responsive manner. Travel as necessary.
Major Responsibilities
- Build and maintain strong relationships with hospital administrators, physicians, nurses, and other healthcare professionals to promote our hospice services.
- Develop and execute a strategic sales plan to generate and increase hospice referrals from hospitals and post-acute care settings.
- Conduct regular visits to hospitals and post-acute care settings to meet with staff, provide educational materials, and offer support to facilitate referrals.
- Collaborate with the hospice team to ensure a smooth admission process for patients referred from hospitals and post-acute care settings.
- Organize and participate in healthcare events, conferences, and seminars to increase awareness of our hospice services within the medical community.
- Monitor and analyze referral trends, competitor activities, and market conditions to identify growth opportunities and adjust sales strategies accordingly.
- Provide timely and accurate reports on sales activities, outcomes, and challenges to the management team.
- Work closely with the marketing team to develop materials, campaigns, and strategies to promote hospice services to hospitals and post-acute care settings effectively.
- Participate in evening/weekend call for admissions as required, in a competent and responsive manner.

Requirements
Education and Experience
- Bachelor’s degree in business, Healthcare Administration, Marketing, or a related field or equivalent experience.
- Proven track record of success in sales, preferably in the healthcare or hospice industry.
- Knowledge of the hospice industry, end-of-life care, and related medical terminology.
- Exceptional communication and interpersonal skills to build and maintain professional relationships with healthcare providers.
- Strong negotiation and presentation skills to effectively promote hospice services and handle objections.
- Ability to work independently and as part of a team, with excellent time management and organizational abilities.
- Willingness to travel frequently to visit hospitals and post-acute care settings within the assigned territory.
- Demonstrated compassion and empathy for patients and their families facing end-of-life challenges.
- Proficiency in using sales and CRM software to track leads, manage accounts, and analyze data.
- Ability to work in an interdisciplinary setting.
Salary : $55,000 - $75,000
