As an Internal Auditor at Transitions Care you will truly make a difference in people’s lives. You will be pivotal in providing unbiased feedback to our teams and ensuring we are adhering to the highest standards of Hospice care.

We are seeking a knowledgeable and detail-oriented Internal Auditor with expertise in CMS Conditions of Participation (CoPs) and healthcare compliance to join our hospice organization. The Internal Auditor will play a critical role in monitoring compliance with federal and state regulations, focusing on the CMS CoPs for hospice care, documentation standards, and operational excellence. This role will also involve conducting on-premises surveys at hospice locations to ensure compliance with regulatory requirements and to assess the readiness for external surveys, inspections, or audits.

Major Responsibilities

- Audit Planning and Execution

  • Develop and execute risk-based compliance audit plans tailored to hospice operations and CMS CoPs requirements.
  • Conduct detailed audits of patient records, documentation, and care plans to ensure compliance with CoPs, including eligibility assessments, interdisciplinary team (IDT) documentation, and patient-centered care planning.
  • Perform audits of on-premises operations, including facility readiness, patient care practices, and administrative procedures.
  • Evaluate hospice location compliance with state and federal licensing requirements and standards.
  • Deliver standardized feedback to administrators, leadership, and staff as to areas of potential improvement.
  • Develop, monitor, and report on plans of correction, ensuring that identified deficiencies are elevated to leadership and addressed in a timely manner.

-CMS and Regulatory Compliance

  • Ensure the organization’s adherence to CMS Conditions of Participation, including:
  • Comprehensive assessments and timely updates to patient care plans.
  • Documentation of patient eligibility for hospice care, including certifications of terminal illness and physician narratives.
  • Interdisciplinary team (IDT) meeting compliance and documentation.
  • Accurate use of the Hospice Item Set (HIS) and data submission requirements.
  • Prepare the organization for external audits, surveys, and inspections, including state and federal site visits.
  • Monitor compliance with HIPAA, OIG guidelines, and other relevant healthcare laws and regulations.

- On-Premises Surveys

  • Conduct routine, announced/unannounced on-premises compliance surveys to evaluate operations, patient care practices, and physical facility conditions.
  • Ensure hospice sites are survey-ready at all times, meeting CoPs and state health department standards.
  • Review staff adherence to patient care protocols, infection control policies, and medication management procedures.
  • Identify gaps in compliance and provide actionable recommendations to address deficiencies.

-Reporting and Recommendations

  • Prepare detailed audit and survey reports that summarize findings, risks, and actionable recommendations.
  • Present audit results and compliance risks to leadership and collaborate on corrective action plans.
  • Track progress on the implementation of corrective actions and re-audit as necessary.

-Training and Collaboration

  • Educate staff on CMS CoPs, best practices for documentation, and compliance requirements.
  • Provide guidance to clinical and administrative teams on preparing for external surveys or inspections.
  • Collaborate with the Compliance Director and leadership to implement organization-wide improvements

Requirements

Education & Experience:

  • Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or related field.
  • Minimum of 3-5 years of experience in healthcare compliance or internal auditing, with hospice-specific experience preferred.
  • Extensive knowledge of CMS Conditions of Participation for hospice care and state health department regulations.

Certifications (preferred but not required):

  • Certified Healthcare Compliance (CHC)
  • Certified Hospice and Palliative Care Administrator (CHPCA)

Skills & Competencies:

  • Strong understanding of hospice operations and CMS CoPs requirements.
  • Experience conducting on-premises compliance surveys and audits.
  • Proficiency in analyzing documentation for compliance with federal and state regulations.
  • Excellent written and verbal communication skills to create clear reports and recommendations.
  • Detail-oriented with strong organizational and analytical skills.


Salary : $75,000 - $85,000
