As a Coordinator of Social Services at Transitions Care you will provide a wide range of psychosocial services to terminally ill patients and their families, including assessing the patient and family psychosocial needs, and pre-bereavement/anticipatory grief needs. Regularly consulting with team social worker to help assess patient’s and family’s psychosocial needs. Travel as necessary.
Major Responsibilities
- Function as support to other members of the ancillary services team
- Function as a support person for the patient/family/caregiver on an ongoing basis
- Function as an expert facilitator of obtaining community resources to meet the needs of patients and families, including financial assistance. Assist families with referrals to community resources when indicated
- Assist patient and/or family with advanced directives
- Assist family/caregiver/patient with placement of patient in a facility for basic care, when appropriate
- Complete and maintain all patient records at patient’s residence and submit accurate and relevant notes regarding patient’s condition and care given
- Assist team social worker in assessing psychosocial and pre-bereavement needs and providing support when needed
- Attend and actively participate in team meetings, staff meetings and in-services, contribute positively to patient care planning
- Work in tandem with other Transitions interdisciplinary team members
- Participate in evening/weekend call as required, conducting on-call services in a competent and responsive manner
- Assure for compliance with local, state and federal laws, Medicare regulations, and
- Actively participate in staff meetings, department meetings, team meetings, briefings, in services, committees and other related activities as needed.
- Adherence to a code of conduct conducive with Transitions Healthcare policy is expected.

Requirements
Education and Experience
- Minimum, age requirement of 18
- Bachelor’s degree in social work, sociology, psychology, or a related human services degree
- Preferred Licensure and/or certification in applicable state
- Minimum of one year of experience in a healthcare setting
- Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).
- Compliance with accepted professional standards and practices
- Understands Hospice philosophy, principles of death/dying
- Ability to work in an interdisciplinary setting
- Physical exam in accordance with state law requirements
- Valid driver’s license and an automobile that is insured in accordance with state and/or Transitions requirements and is in good working order
- Satisfactory references from employers and/or professional peers
- Satisfactory criminal background check
- Demonstrate excellent observation, judgment and communication skills. Ability to provide good written documentation in a timely manner
- Self-directed with the ability to work in conjunction with other team members
Salary : $55,000 - $65,000
