As a Social Worker at Transitions Care you will provide a wide range of psychosocial services to terminally ill patients and their families, including regularly assessing the patient and family psychosocial needs, and pre-bereavement / anticipatory grief needs. Implements the Plan of Care provisions to meet their needs, which include; social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care. Local Travel, as necessary.
Major Responsibilities
- Conduct initial psychosocial, spiritual, and pre-bereavement assessment of patient / family / caregiver in a thorough, accurate, compassionate, and timely fashion.
- Function as a support person for the patient / family / caregiver on an ongoing basis.
- Function as expert and facilitator of obtaining community resources to meet the needs of patients and families, including financial assistance. Assist families with referrals to community resources when indicated.
- Assist family with Advanced Directives.
- Assist family / caregiver in placement of patient in a facility for basic care, when appropriate.
- Complete and maintain all patient records at patient's residence and submit accurate and relevant notes regarding patient's condition and care given.
- Attend and actively participate in team meetings, IDG meetings, and in-services, and contribute positively to patient care planning.
- Assist patient / family / caregiver and other team members in providing continuity of care.
- Participate in evening / weekend call as required, conducting on-call services in a clinically competent and responsive manner.
- Maintain clinical records, statistics, reports, and records for the purpose of evaluation and reporting of agency activities as prescribed and in compliance with local, state, and federal laws.
- Maintain a caseload and deliver care in a cast-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
