The LPN Wellness Coordinator will be responsible for all nursing-related components of the admission process. This includes, but is not limited to, conducting nursing assessments, verifying and transcribing admission orders, and ensuring medications arrive in a timely manner.
LPN Wellness Coordinator Job Highlights:
- Assist charge nurse with floor duties as needed.
- Ensure all labs are drawn and all results are sent to the Primary Care Physician (PCP) per physicians orders.
- Educate Licensed Practical Nurses (LPN’s) on chart organization and ensure charts are maintained
- Provide tours of the Quarters as needed to perspective customers.
- Assess residents in emergency situations and assist with hospital transfers if needed.
- Monitor residents’ conditions and treatment plans while they are in the hospital.
- Help residents complete applications for medically-related services (e.g. PACE/ PACENET). This excludes Power of Attorneys, Living Wills/POST Forms, and Last Will and Testaments.
- Help the nightshift Licensed Practical Nurse (LPN) order medications to ensure they arrive in a timely manner.
- Complete all medical pre-authorizations (ex. Xray,Ultrasound)
- Schedule resident’s appointments.
- Help find coverage on a daily basis for open Personal Care Aide (PCA ) and Licensed Practical Nurses (LPN) shifts.
LPN Wellness Coordinator Minimum Qualifications:
- Graduation from an accredited school of nursing as a Licensed Practical Nurse or Registered Nurse.
- Current LPN or RN licensure by PA State Board of Nursing or Multi State Licensure
- CPR certification (prior to or upon employment)
