POSITION TITLE: Housing Targeted Case Manager
SUPERVISOR: Director of Recovery Housing
QUALIFICATIONS:
- Bachelor’s degree in behavioral science or related area or; Equivalent combination of experience and training and eligible for Kentucky Targeted Case Management Training preferred
- Case management experience, preferably with individuals with substance use disorders
- Peer Support Certification preferred
- Familiarity with Substance Use Disorders and Mental Health Disorders
- Comfortable in a home visit setting and interacting with people of diverse backgrounds and abilities
- Detail oriented and thorough documentation skills
- Excellent listening and information gathering skills; effective problem solving
- Self-motivated with ability to take initiative, as well as work as part of a team
- Sound mental reasoning, sound judgment, and the ability to respond calmly and effectively in a crisis situation is required.
JOB SUMMARY:

The Housing Case Manager will be responsible for reviewing housing applications; interviewing housing applicants; completing Participant Agreement paperwork for new program participants; maintaining program participant files in Credible with a minimum bi-weekly case note; collecting program fees; conducting random UA/BA; inspecting rooms and houses weekly; attending monthly house meetings; creating long term housing plans including budgets; evictions and discharges, as needed.
AGENCY REQUIREMENTS:

ESSENTIAL JOB ELEMENTS:
- coordinate emergency efforts in case of fire, tornado, etc.
- communicate with clients, other staff, staff from referral agencies, the public
and emergency personnel.

- remain aware and cognizant of participant activity and activities within the houses.
- willingness to be called in or work non-traditional hours in order to respond to
significant issues, emergencies or crises.
RESPONSIBILITIES
- Case Management:
- Review housing applications; interview applicants for appropriateness and eligibility. Complete Participant Agreement paperwork.
- Perform weekly case management sessions with each participant and document progress in Credible.
- Collect program fees and document appropriately. Write receipts and turn fees collected into the Director of Recovery Housing weekly.
- Inspect rooms/houses regularly.
- Attend the required training and meetings.
- Monitor participants and activities in each building.
- Watch for building maintenance needs and respond timely with referral to Director of Recovery Housing.
- Assist in participant stability by referring to other needed community services and mainstream resources.
- Represent Transitions in a positive and professional manner.
This is a full-time (40 hours during normal business hours), salaried position. The benefits package includes holiday, vacation, and sick pay, as well as health insurance, dental insurance and retirement. The position will involve local travel around Covington; therefore, a valid driver’s license, auto insurance and reliable vehicle are required. Mileage reimbursement is provided. Possible exposure to communicable diseases, emotionally stressful working conditions, and irregular hours.
