What are the responsibilities and job description for the Nurse Liaison position at TRANSITIONS LIFECARE?
The Nurse Liaison is a full-time nursing position focused on coordinating referrals, admissions, and care transitions across all TL service lines within hospital and community-based settings. This role works closely with hospital staff and leadership to ensure timely, appropriate patient care and serves as a case manager for hospice patients under the general inpatient level of care. The Nurse Liaison also provides education and support to patients, families, and referral sources, assisting with goals of care discussions and promoting TL services. Additional responsibilities include training hospital staff, addressing patient and referral source concerns, and collaborating with TL Provider Relations to enhance outreach efforts. The schedule for this position is Monday-Thursday 8:30am-7:00pm; weekends as needed; 1 major and 1 minor or 3 minor holidays per year.
Requirements:
- Active RN license in NC (BSN preferred)
- Minimum of 2 years recent clinical experience (hospice/home health/nursing home preferred)
- Strong critical thinking, organizational, and communication skills
- Experience with electronic medical records and Microsoft Office preferred
- Current CPR certification and valid NC driver’s license with reliable transportation
- Ability to travel within TL’s service area and lift/carry up to 50 pounds
