What are the responsibilities and job description for the On-Call RN, Full-time position at TRANSITIONS LIFECARE?
Transitions LifeCare is seeking a compassionate and skilled On-Call RN to provide after-hours nursing support to patients and families. This full-time role includes home visits to manage urgent needs, provide symptom relief, and support patients and families during end-of-life care. The On-Call RN works closely with the Manager on Call and Triage Nurse to ensure timely, high-quality care during evenings, weekends, and holidays. The schedule for this position is: 3 (12) hour night shifts per week.
Key Responsibilities:
- Provide after-hours clinical care, symptom management, and emotional support to hospice patients and families.
- Respond to patient and family concerns, including end-of-life situations, through in-person visits.
- Assist triage staff in covering patient needs by phone or in the field.
- Document care and interventions in compliance with CMS CoPs and organizational standards.
- Collaborate with the interdisciplinary team to coordinate care and level-of-care determinations.
- Supervise LPNs and Hospice Aides as needed.
- Attend at least 4 team meetings annually.
Requirements:
- Current NC RN license; BSN and CHPN preferred.
- 3 years recent clinical experience; hospice, home health, or long-term care preferred.
- Strong clinical judgment, time management, and computer skills.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Must be available to work assigned weeknights, weekends, and holidays.
- CPR certification required.
