We’re seeking a Patient Care Coordinator (LPN) to provide vital support to our hospice team. In this role, you’ll assist the Hospice Team Leader and clinical staff with key administrative and clinical tasks. Your responsibilities include managing electronic medical records, processing physician orders, tracking prescriptions, and coordinating patient care updates. You’ll ensure documentation accuracy, regulatory compliance, and effective communication between patients, families, and healthcare providers.
Key Responsibilities
- Enter patient data and update records in Allscripts and Netsmart systems.
- Manage physician orders, ensuring timely signatures and follow-up.
- Process admission orders and medication requests.
- Coordinate patient care issues and communicate with clinical teams, families, and providers.
- Oversee staff schedules and participate in interdisciplinary meetings.
- Assist with compliance, quality improvement processes, and monthly billing audits.
Requirements
- Active Licensed Practical Nurse (LPN) license.
- 1–2 years of healthcare experience with scheduling and multitasking.
- Proficiency in electronic medical records, Microsoft Word, and Excel.
- Strong organizational and communication skills.
- Preferred: Experience with 485s and Medicare documentation.
