Transitions LifeCare is seeking a results-driven and relationship-oriented Provider Relations Representative to support the growth of our Hospice and Palliative Care programs. This full-time role is responsible for educating referral sources, building strong partnerships, and facilitating timely, appropriate referrals. The position will serve as a key liaison between TL and physician practices, hospitals, and long-term care communities. The schedule for this position is Monday-Friday 8:00am-5:00pm.

This full-time role is responsible for educating referral sources, building strong partnerships, and facilitating timely, appropriate referrals. Key Responsibilities
- Increase referrals and census by promoting TL’s services.
- Develop and manage a territory-based referral account list.
- Conduct regular visits to accounts based on referral potential.
- Educate providers and care teams on hospice and palliative care benefits.
- Coordinate patient information visits and referral follow-up.
- Provide in-services and support health fairs/trade shows.
- Collaborate with clinical staff to address concerns and ensure seamless referral processes.
- Document activities in CRM and uphold high customer service standards.
- Bachelor’s degree preferred.
- Experience in hospice, healthcare, or senior care settings.
- Strong communication, interpersonal, and organizational skills.
- Marketing or sales experience a plus.
- Comfortable with occasional evenings/weekends.
- Must be self-directed, collaborative, and proficient with technology.
- Ability to lift up to 30 lbs and travel using a personal vehicle (mileage reimbursed).
