Job Description
As a Case Manager, this role provides on-site case management services in an apartment building setting for residents who have transitioned from homelessness, engaging residents individually and within the community using trauma-informed and harm-reduction approaches to promote stability and independence.
Experience
Required:
• BA in Psychology, Social Work, or related field.
• Minimum of two years direct service/case management and/or chemical dependency counseling.
• Experience working with co-occurring diagnosed clientele.
• Experience with crisis management and intervention.
• Experience in case management with diverse populations.
• Ability to successfully pass a criminal background check.
• Possess valid driver's license and vehicle insurance.
Preferred:
• MA, MSW in education, social services, or related field or 5 years' experience in program development and staff supervision.
• Experience with grant writing, public relations, fundraising, volunteer coordination, and Board relations.
Key Responsibilities
- Provide direct service and referrals.
- Enter and maintain accurate data in the Community Management Information System (CMIS) and run monthly & quarterly reports.
- Enter and maintain accurate service records including releases and documentation for Foundational Community Support (FCS) billing.
- Collaborate with community service providers.
- Work cooperatively with AmeriCorps members, interns, and other volunteers.
- Be a mandated reporter of child and adult abuse.
- Participate in staff engagements which further the Mission and Values of Transitions.
- Maintain a safe environment for program participants.
- Build relationships and community among the participants.
- Facilitate or co-facilitate recreational, support or life-skills groups, depending on need and area of expertise.
- Address the daily operations of the program.
- Within the context of the stated primary job duties, and consistent with Transitions fundraising and marketing efforts, serve as ambassador and positive representative of Transitions' mission, values, and programmatic efforts.
