Position Overview:
- Role: Conduct annual health assessments for geriatric patients in our new Wellness Clinic
- Work on site at a Clinic in the Visalia area

Your Responsibilities:
- Perform Medicare Annual Health Assessments for geriatric patients.
- Conduct patient histories and physicals, using preventative health screening tools.
- Document patient information using EMR systems.
Requirements:
- Current NP license for California.
- Experience with geriatric patients and EMR systems.
- Comfort with electronic documentation.
