SUMMARY
The RN Care Manager for the Recovery Program/Substance Use Disorder Treatment (SUD TX) program will be part of a team of health professionals working with people with substance use disorders.
CORE FUNCTIONS
- Performs duties and health services consistent with education, training, licensure and certification, consistent with treatment plans outlined for patients by providers.
- Works very closely with the Substance Use Disorder Treatment team which includes primary care providers, behavioral health providers and a peer recovery navigator.
- Develops, implements and evaluates individualized patient care plans in a person-centered, patient driven manner.
- Serves as a liaison between providers, patients, families, collaborative service networks and other services.
- Confers with peer recovery navigator with assessment, monitoring and evaluation of patient rehabilitation and adherence to this program.
- Provides nursing responsibilities required in substance use disorder treatment programs, including dosing, documentation and other required activities.
- Provides monthly review of care plans in conjunction with the behavioral health provider and peer recovery coach to monitor patient success.
- Provides education on health conditions, treatment recommendations, medications and strategies to implement care plan goals including both clinical and non-clinical needs.
- Communicates with medical providers, subspecialty providers including mental health and substance abuse service providers, long term care and hospitals regarding records.
- Ensures all elements of treatment programs including progress towards goals, medication dosage, toxicology screens, and other parameters are appropriately entered into eCW (EMR) and that all data can be analyzed and reported.
REQUIREMENTS
- Current license as Registered Nurse or LPN in the State of Michigan.
- Bachelors Degree in Nursing preferred.
- Current BLS certification and ability to use AED.
- Prior RN/LPN experience and proven ability to work as a team member.
- Excellent care management skills.
- Excellent interpersonal skills and ability to work with diverse populations of varying socio-economic backgrounds.
