Responsibilities

We are seeking a Physical Therapist to provide intermittent home health, palliative, hospice, and custodial care services in the Chicagoland area. Responsibilities include:

  • Assessing and evaluating patients' therapeutic and functional status
  • Developing comprehensive care plans
  • Instructing patients and caregivers on therapeutic appliances
  • Determining priority needs for physical therapy

Qualifications

  • Baccalaureate or master's degree in physical therapy from an accredited program
  • Current Illinois Physical Therapist license
  • Current CPR certification
  • Valid Illinois driver's license and auto insurance
  • Home Care Home Base experience preferred
