What are the responsibilities and job description for the Community Inclusion - RN position at TRC Community Health Center?
TITLE: REGISTERED NURSE (RN)
RESPONSIBLE TO: Executive Director
FUNCTION: To consistently provide high-quality nursing services within the Primary Care setting. To provide assessment, screening, healthy lifestyle support, education and chronic disease management with a goal of improving health outcomes and facilitating access to services.
SPECIFIC DUTIES:
1. Monitoring patient’s condition and assessing their needs to provide the best possible care and advice within NYS RN Scope of Practice : https://www.op.nysed.gov/professions/registered-professional-nursing/practice-information
2. Observing and interpreting patient’s symptoms and communicating them to physicians
3. Collaborating with physicians and nurses to devise individualized care plans for patients
4. Perform routine procedures (blood pressure measurements, administering injections etc.) and document in patients’ charts as per defined parameters.
5. Adjust and administer patient’s medication and provide treatments according to physician’s orders
6. Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing exam tables etc.)
7. Provide instant medical care in emergencies
8. Foster a supportive and compassionate environment for patients
9. Provide assessment, treatment, care and timely follow-up as directed by provider order, established treatment plans, and/or as warranted by medical necessity.
10. Work with the primary healthcare team to provide patients with primary medical care services within the standard of care for patients.
11. Work collaboratively with the healthcare team to ensure positive and effective patient experiences.
12. Evaluate medical needs as they arise and provides nursing care or recommends referral for appropriate medical services.
13. Collaborate with healthcare team to create, document, implement and evaluate individualized care plans for patients.
14. Available for walk-in appointments and emergency situations, as needed.
15. Provide emergency/acute evaluation and intervention.
16. Ensure control of communicable disease and infection through identification and assessment, reporting to medical authorities and implementing protective and preventative measures.
17. Perform and document routine procedures (vitals, vaccinations, treatment of injuries, etc.).
18. Records complete, timely and legible medical records.
19. Maintain current knowledge of nursing care practices.
20. Possess excellent communication and inter-personal skills.
21. Provide advice to patients for lifestyle and diet changes that may improve their health or help to treat the heath issue they are experiencing.
22. Serve on committees as needed or assigned.
23. Maintain patient confidentiality and complies with all federal and state health information privacy laws.
24. Adhere to departmental policies, procedures and objectives, ongoing quality improvement objectives and safety, environmental, and infection control standards.
25. Expand knowledge and capabilities by attending educational workshops, conferences etc
26. Provide high quality patient care and customer service
27. Perform all other duties as needed or assigned.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Must possess strong nursing skills and abilities in order to provide comprehensive care on a daily basis as well as in emergency situations; A firm understanding of current and relevant nursing/medical policies/procedures/practices is essential, Must possess extensive knowledge of all applicable state and federal healthcare regulations (i.e. HIPAA, etc.); Must possess strong organizational and time management skills; Must be self-directed and able to initiate and manage multiple responsibilities effectively and efficiently; Excellent verbal and written communication skills across various channels is paramount; Experience with individuals with disabilities is helpful; Ability and willingness to learn and utilize new technology (i.e. EHR, etc.);; Sound technical abilities as related to routine and complex nursing tasks, included, but not limited to – g-tubes and principles of oxygen administration; Must be flexible and willing to work non-traditional hours as needed or assigned to meet patient need; Physical condition or reasonable accommodation(s) commensurate with the demands of the job; Ability to lift a minimum of 50 pounds waist high is required.
MINIMUM QUALIFICATIONS: Bachelor’s Degree in Nursing, or related field, with New York State licensure to practice as a Registered Nurse (graduates must obtain licensure within 90-days of offer and must work under the direct supervision of licensed RN Manager), plus one (1) year of experience serving in a nursing capacity (excluding graduates); OR Associates Degree in Nursing, or related field, with New York State licensure to practice as a Registered Nurse (graduates must obtain licensure within 90-days of offer and must work under the direct supervision of licensed RN Supervisor or RN Manager), plus three (3) years of experience serving in a nursing capacity (excluding graduates).
This is a position that is exempt from overtime provisions of the Fair Labor Standards Act. As an Exempt employee, incumbent will receive an annual salary commensurate with assigned responsibilities and salary classification; however, incumbent is expected and required to work as many hours as necessary to fulfill job responsibilities.
