Day in the Life of a Patient Navigator
Greeting patients and answering clinic phones.
Assist patients in understanding their treatment options, and the resources.
Serve as an essential link between patients and all other care.
Facilitate appointments for consults and support services within established service standards, including social workers, physician, etc.
Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management.
Work with staff and others to collect data, track outcomes, and support strategic planning processes.
Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, social services.
Assist Center in stocking supplies and Patient Ambassador team.
Other duties as assigned.
Education and Experience requirements:
Must hold one of the following credentials/experiences:
RN, LPN, or BSW
Bachelor’s Degree WITH at least 2 years of lived addiction experience.
Associate's degree or CMA WITH 4 years of lived addiction experience.

Mileage reimbursement: Crossroads matches the current IRS mileage reimbursement rate.

Tuesday - Hamburg 2:30pm-7:15pm
Wednesday - Stanhope 10:00am-2:00pm
Thursday - Belvidere 11:30am-5:00pm
Sunday - Stanhope 9:00am-1:00pm
