POSITION PURPOSE:
The Case Manager is responsible to affect a safe, comfortable and supportive environment for the patients participating in the program. These duties include and are not limited to intake, initial assessment, initial treatment planning, administrative tasks/projects, discharge summaries, individual assessments with clients and consultation with other professionals in regard to client treatment and services. The Case Manager will work effectively and cooperatively with the Clinical staff to support clients to abstain from substances and live happy and productive lives free of their addiction.
ESSENTIAL JOB FUNCTIONS:
- Accountable for leading a WE CARE culture that focuses on our mission to provide a transformational influence to patients and their loved ones.
- Monitors each patient throughout the daily schedule.
- Enforces program rules and expectations and reports infractions to the Therapist and Clinical Director.
- Monitors patient conformance with program schedule, reports problems and informs supervisor.
- Makes daily assignment checks and discusses results with each patient.
- Monitors patient participation and reports problems to supervisor.
- Resolves conflicts through general and supportive advice.
- Participates in patient recreation, leisure, and weekend activities as scheduled.
- Proves case management services.
- Transports patients to scheduled activities or arranges transportation.
- Reports any vehicle problems to supervisor.
- Prepares educational and motivational lectures and groups.
- Monitors AA/NA meetings on premises including recruitment of speakers.
- Maintains the confidentiality of all patient and facility activities.
- Complies with Our Company Policies and Procedures.
- Accepts and utilizes supervision 16. Addresses problems noted by supervisor.
- Performs other assignments as requested by supervisor.
- Demonstrates willingness to accept responsibility.
- Completes all tasks with concern for work quality
POSITION REQUIREMENTS & QUALIFICATIONS:
EDUCATION: Graduation from High School or GED preferred.
EXPERIENCE: At least six (6) months experience working in the Human Services field
LICENSURE/CERTIFICATION: Maintains current CPR certification.
KNOWLEDGE, TRAINING, FUNCTIONAL COMPETENCIES REQUIRED AT TIME OF HIRE:
- Knowledge of the Principles and Practices of the discipline.
- Knowledge of Joint Commission Standards.
- Demonstrates Proficiency in Communication & Written skills.
- Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws.
- Knowledge of Drug-Free Workplace Policies.
- Knowledge of Workplace Violence.
