Job Description: As a Case Manager in Addiction Treatment, you will play a crucial role in supporting and guiding individuals and their families throughout their recovery journey. Your exceptional skills in assisting patients through the recovery process, helping with attorneys, probation officers, FMLA, Short Term Disability, interventions, family engagement, and continuum of care will contribute to the development and implementation of comprehensive treatment plans, ensuring the highest quality of care and support for our Patients.
Responsibilities:
- Conduct thorough case management assessments of patients' addiction history, treatment needs, and psychosocial factors to inform treatment planning.
- Utilize exceptional intervention techniques to engage and motivate individuals struggling with addiction, encouraging them to actively participate in treatment.
- Collaborate with treatment team members to develop individualized treatment and continuing care plans based on clients' unique needs and goals.
- Provide ongoing case management services, including regular check-ins, progress monitoring, and adjusting treatment plans as necessary.
- Facilitate family engagement by involving and educating family members about addiction, recovery, and their role in supporting their loved one's journey.
- Coordinate and advocate for comprehensive continuum of care, ensuring patients have utilize all White Light support services, and aftercare planning.
- Collaborate with external stakeholders, such as community organizations and healthcare professionals, to establish a network of support for patients and their families.
- Maintain accurate and timely documentation of patient progress, treatment plans, and interventions in compliance with organizational standards and regulatory requirements.
- Stay informed about the latest developments in addiction treatment, interventions, and best practices, continually enhancing your knowledge and skills through professional development opportunities.
Qualifications:
- Bachelor's psychology, social work, counseling, or a related field preferred but not required.
- Certification as a CDCA is required.
- Minimum of 5 years of experience working in addiction treatment or related fields.
- Proven expertise in interventions, family engagement, and continuum of care.
- Strong understanding of addiction and recovery processes, evidence-based treatment modalities, and the impact of addiction on individuals and families.
- Strong understanding of the principles of AA and the roadmap of recovery.
- Excellent interpersonal and communication skills, with the ability to empathize, motivate, and build rapport with individuals from diverse backgrounds.
- Strong organizational and time management skills, with the ability to handle multiple cases and priorities effectively.
- Knowledge of, or ability to learn and follow, relevant laws, regulations, and ethical guidelines governing addiction treatment and confidentiality.
- Proficient in documentation and record-keeping, with attention to detail and accuracy.
- Compassionate, non-judgmental, and dedicated to providing exceptional care and support to patients and their families.
