Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by assessing their needs, developing comprehensive care plans, and facilitating access to necessary resources. This position requires strong interpersonal skills, the ability to work collaboratively with various stakeholders, and a commitment to improving the lives of clients through effective case management practices.
Responsibilities
- Conduct thorough assessments of clients’ needs and circumstances to develop tailored service plans.
- Coordinate and connect clients with appropriate services, including healthcare, housing, and social support.
- Provide crisis intervention and management to clients facing immediate challenges.
- Monitor client progress and adjust care plans as necessary to ensure optimal outcomes.
- Collaborate with community organizations, healthcare providers, and government agencies to facilitate service delivery.
- Maintain accurate and up-to-date documentation of client interactions and case progress.
- Advocate for clients’ rights and access to services, including navigating Medicare and Section 8 housing assistance.
- Provide support in areas such as behavioral health, addiction counseling, child welfare, and workforce development.
- Supervise interns or junior staff members as needed.
Qualifications
- Bachelor’s degree in Social Work or a related field; Master’s degree preferred.
- Experience in crisis intervention, behavioral health, or addiction counseling is highly desirable.
- Familiarity with Medicare processes and child welfare systems is a plus.
- Strong crisis management skills with the ability to remain calm under pressure.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a multidisciplinary team.
- Proficient in case management software and Microsoft Office Suite.
- A valid driver’s license may be required for client visits.
