Trend Health & Rehab of Meridian is a short-term rehab and long-term care facility in the Meirdian Area!

Our facility located in Meridian, Mississippi. We are a long term care facility specializing in short term rehab, and long term placement of residents.

We are looking currently for 8AM to 4:30PM RN for MDS Coordinator Monday through Friday.

Job Summary:

MDS coordinator: A minimum data set (MDS) coordinator is a nursing professional who helps manage a nursing team in the nursing home. MDS coordinators are registered nurses or licensed practical nurses who receive advanced training to prepare for this senior role. The MDS coordinator monitors patient care by assessing procedures, speaking with patients and recording medical codes. MDS coordinators are often also responsible for creating medical codes, which allow hospitals and clinics to coordinate with medical billers and keep accurate records.

The main job duty of an MDS coordinator is to manage patient care, so they have several responsibilities that they perform to help ensure quality care. Here are some job duties an MDS coordinator may do:

Develop goals for improving treatment and care plans

Evaluate the patient care that long-term patients receive

Interview new patients to assess the quality of care they're receiving

Meet with patient caretakers and families to discuss the patient's condition and treatment plans Record data from patients' medical charts

Approve resident applications for long-term care facilities

Monitor patients' medical insurance and payment plans

Host meetings to discuss patient care and implement new strategies

The Registered Nurse/Licensed Practical Nurse is responsible to the Director of Nursing.

Education:

He/She must be a graduate of an accredited school of nursing currently registered with the state agency for nursing licensure and hold a valid nursing license in the state he/she is employed.

Qualifications:

· Demonstrates leadership and managerial ability.

· Has the ability to organize, plan and supervise care and the Nursing Assistants assigned to work a designated unit and shift.

· Maintains effective interpersonal relationships.

· Is of good emotional, mental and physical health, having sound judgment and high professional standards.

· Must be willing and capable of providing emergency assistance to residents as indicated.

· Is courteous in working with resident, families and employees.

· Maintains a neat, well-groomed and professional appearance at all times.

· Has a desire to work with the geriatric resident and their current and prospective needs.

· Must be will to be in the facility ready to begin assigned duties on time.

· Must be willing to be in the facility during weekdays as well as weekend hours.

· Must be will to work preferred shifts as scheduled and assist on other shifts needed to assure adequate staffing to meet the needs of each resident.

· Must demonstrate ability to evaluate the geriatric resident’s needs and provide intervention timely to assure each resident’s needs are met.

· Must demonstrate ability to evaluate nursing staff needs and provide staff education timely to provide quality of care.

· Must demonstrate ability to function as a cooperative team member with each member of the Nursing Department and other departments.

· Must demonstrate the ability to manage and supervise assigned unit in a productive, timely manner.

· Must have a pre-employment and an annual Tuberculosis screening examination.

Responsibilities:

Resident Care

· Continually assess and reassess residents for risk or complications as a result of being in the facility. (i.e., skin breakdown, incontinence, contractures, reduced mobility, reduced involvement in bathing, dressing, or eating, weight loss, behaviors due to change in lifestyle, etc.) Documenting as necessary to communicate complications.

· Makes rounds of the assigned unit frequently during the shift providing intervention as needed. During the entire shift, will be attune to needs of the unit while doing other nursing tasks.

· Prepares and administers medications as ordered. Reorders medications as needed in preparation for future shifts. Assess, communicates, and documents medication issues as per policy. May have to communicate to physician of changes of condition which could lead to new orders which are inputted and ordered by the nurse.

· Provides special procedures as ordered.

· Completes the treatments as ordered. Asses and documents new and old areas of treatment describing in detail the results of staff action and the results of the treatment. May assist in inspecting residents for skin break down and document in the wound assessment manager and/or skin inspection report. Contacts the physician for any areas not responding positively and timely.

· Supervises the meal services daily, Works with the staff to assure each resident is provided adequate nutrition for each meal and ensure that resident is being served the correct ordered diet.

· Document on nurse’s notes as per policy. Assures there is accurate and timely documentation of any unusual observation, incident, or happening.

· Reviews information, reassess residents, interviews staff, reviews documentation of each shift and writes monthly nurse progress note using the Interdisciplinary Care Plan documenting progress or lack of progress toward the resident’s goals. Writes suggestions for changes to be made to the plan of care on the “Working Copy” of the Interdisciplinary Care Plan.

· Admits residents as required. Ensures medications for new admissions are ordered and that a quick and efficient admission occurs.

· Assists with auditing charts.

· Assists with answering the telephone and communicating on behalf of the facility.

· Reviews and updates Medication Administration Record, Treatment Administration Record, and the Cursing Care Flow Records for the upcoming month.

· Makes rounds toward end of shift to assure area is in order for the oncoming shift.

· Counts required medications with oncoming and off going shifts. Ensures emergency drug kit is accounted for each shift.

· Monitors storage of drugs which includes cart organization and cleanliness and monitoring temperatures of drugs needing to be refrigerated.

· Keeps nurse’s station, medication room clean. Assigns Nursing Assistants to keep each ancillary room clean and orderly. Assigns staff to keep equipment clean and proper condition for use.

· Ensures residents orders are followed which includes Full Code and Do Not Resuscitate orders.
