SUMMARY OF POSITION
The Admission Coordinator is responsible for ensuring that anyone being admitted to Allen Morgan Health Care (AMHC) meets the criteria for admission to either the skilled nursing facility (SNF) or for intermediate care (ICF) at AMHC. The Admission Coordinator is responsible for the coordination of tours in the facility with prospective consumers of services being offered at the facility and following up with consumers seeking placement for someone in AMHC requiring long term or skilled care.
The job responsibilities include obtaining referral documentation from providers and then presenting the information to the admissions committee to consider whose needs can best be met by the facility. The admissions committee can then review and make an informed decision regarding the appropriate placement of the resident in the nursing home.
The Admission Coordinator is responsible for marketing the services offered in Allen Morgan Health Care.
PRIMARY DUTIES & RESPONSIBILITIES
- Work in conjunction with resident’s family members, physician and referring agency to obtain critical medical records and documentation for admission into AMHC.
- Able to perform administrative tasks regarding referral processing, resident registration, verification of benefits, insurance authorizations, or any other prior approval needed in the admission process.
- Responsible for completing accurate documentation and records of referrals and referral sources.
- Ensures complete and accurate admission information, including demographics, current medical record information and discharge summary from the sending facility.
- Meet with each prospective AMHC resident or family as needed, review health related and financial information, including medical records and supporting documents for completeness and accuracy.
- Work with physicians and office staff, hospital discharge planners, and family members, to coordinate a resident’s hospital discharge to AMHC on the day of admission.
- Contacts insurance companies to ensure verification of benefits and prior authorization approval requirements are met; ensures that all referrals are addressed in a timely manner.
- Assemble information regarding resident’s clinical record, verbal orders and all documentation are complete prior to admission.
- Stay up to date on changes in the admissions process required for Medicare and long-term care facilities in the State of TN; Become proficient on rules regarding admissions, transfers and discharge of residents into and out of SNF or ICF facilities.
- Attend in-service seminars as needed to develop competency and maintain skills.
- Coordinate and cover services for the admission of residents to the Trezevant Terrace Assisted Care Living Facility (ACLF).
- Become knowledgeable and serve as a back-up staff person to give tours of facility to walk-in consumers who do not have an appointment but want to tour AMHC.
- Maintains excellent communication with referral sources in order to foster good relations and increase census.
- Be knowledgeable of Trezevant’s Mission, vision and values; Always wear appropriate uniform and clean, presentable and professional.
- Other duties as assigned/ required.
License/ Qualifications:
- Maintain a State of TN or a multi-state license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Minimum of three (3) years nursing experience. Experience needed in geriatrics, SNF and admissions criteria.
- 1-2 years’ experience preferred working in a dedicated admissions department in a SNF.
- Must show proficiency with computers and have great communication skills.
