Responsibilities

• Conduct thorough patient assessments, including taking vital signs and evaluating medical histories.
• Perform triage duties to prioritize patient care based on urgency and medical need.
• Develop and implement individualized treatment plans for patients across various settings.
• Provide care for patients in inpatient facilities, nursing homes, and occupational health environments.
• Manage discharge planning processes to ensure smooth transitions from hospital to home or other care settings.
• Utilize telehealth technologies to provide remote patient consultations and follow-ups.
• Prescribe medications and manage treatment regimens as appropriate.
• Collaborate with interdisciplinary teams to enhance patient care quality and safety.
• Educate patients and families on health management strategies and preventive care.

Experience

• A valid Nurse Practitioner license is required.
• Experience in ventilating patients or working in critical care settings is preferred.
• Familiarity with durable medical equipment (DME) management is a plus.
• Previous experience in nursing homes or occupational health environments is advantageous.
• Proficiency in telehealth practices is desirable.
• Strong communication skills and the ability to work collaboratively within a healthcare team are essential.
