Responsibilities
- Conduct thorough patient assessments, including taking vital signs and evaluating medical histories.
- Perform triage duties to prioritize patient care based on urgency and medical need.
- Develop and implement individualized treatment plans for patients across various settings.
- Provide care for patients in inpatient facilities, nursing homes, and occupational health environments.
- Manage discharge planning processes to ensure smooth transitions from hospital to home or other care settings.
- Utilize telehealth technologies to provide remote patient consultations and follow-ups.
- Prescribe medications and manage treatment regimens as appropriate.
- Collaborate with interdisciplinary teams to enhance patient care quality and safety.
- Educate patients and families on health management strategies and preventive care.
Experience
- A valid Nurse Practitioner license is required.
- Experience in ventilating patients or working in critical care settings is preferred.
- Familiarity with durable medical equipment (DME) management is a plus.
- Previous experience in nursing homes or occupational health environments is advantageous.
- Proficiency in telehealth practices is desirable.
- Strong communication skills and the ability to work collaboratively within a healthcare team are essential.
