What are the responsibilities and job description for the IDD Client Services Specialist position at TRI-COUNTY BEHAVIORAL HEALTHCARE?
The IDD Client Services Specialist qualifies as a QIDP, and provides assistance to eligible individuals in gaining access to medical, social, educational, and other needed services that will help the individual achieve a quality of life and community participation acceptable to the individual and/or LAR on the individual's behalf. In addition, the IDD Client Services Specialist provides technical support to HCS & TxHmL Waiver Case Managers, and others as needed. This position's duties include, but are not limited to the following:
- Assists case managers with client transfers in and out of the agency's catchment area
- Assists case managers with TMHP Troubleshooting until all issues are resolved
- Develops an individual plan of care which documents the delivery of appropriate services and supports based on the individual's choices and preferred outcomes
- Completes CFC (Community First Choice) functional assessments
- Works in accordance with MCO's (Managed Care Organizations)
- Will have a reduced caseload commiserate with the needs of the clients in services, and as directed by Program Administrator
- Will help foster a positive work culture and exhibit professional, mature behavior, and work collaboratively with external stakeholders, such as MCOs
- Completes all required documentation, on time and accurately, in the agency's Electronic Health Record (EHR)
- Links, refers, and coordinates the services offered through Tri-County Services and community resources
- Monitors progress monthly to ensure quality and appropriateness of services
- Provides advocacy and crisis prevention/management as needed
- Assists with enrollments and intakes, as needed
- In and out of region travel required
Position Requirements:
- A high school diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
- An Associate degree in a social, behavioral, human service, or health-related field including, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice; or
- Bachelor's or advanced degree from an accredited college or university (ANY field)
- For Bachelor's and Associates degree candidates, at least one year's paid or unpaid experience working with individuals with IDD is preferred
- Bilingual in Spanish & English a plus
- Must have a valid Driver's License and be insurable through Tri-County Behavioral Healthcare.
