Description
Tri County Care Management Organization (CMO) is a non-profit organization that provides care management (case management) services for children with complex behavioral, developmental or addiction challenges in Hunterdon, Somerset and Warren counties. We partner with families and communities to help youth succeed at home, in school and in the community.
Position Summary
Effectively provides direct supervision and leadership to care management staff in fulfilling their responsibilities to designated youth and families. Understands and reflects the core values of the CMO / Wraparound process and other related theoretical approaches.
Essential Functions (including but not limited to)
Assist with translation of documents and verifying translations in Spanish
Supervises team of Care Managers and overseas operations of the team to ensure compliance with standards of care and wraparound fidelity.
Utilizes Quality Assurance tools for adherence to best practice standards and agency policies.
Leads team meetings, provides regular supervision to Care Managers and participates in agency meetings as required.
Assists Care Managers, in consultation with the agency leadership, in identifying and formulating strategies to overcome barriers to service delivery.
Adheres to current Children’s System of Care standards, policies, and regulations.
Collaborates with Care Managers on all aspects of Care Manager duties including leading and scheduling Child & Family Team meetings and preparation of crisis plans, and Individualized Service Plans.
Supports Care Managers in the field at family and community meetings.
Discusses and collaborates on authorizations for provider services for youth and family.
Seeks and recommends community support and services for youth and families served by the Tri County CMO.
Monitors the entry of all required information in the electronic health record agency youth file.
Actively participates in formal and informal quality review efforts as appropriate.
Participate in after hours and weekend on-call Supervisor rotation to provide back up support to the on- call Care Manager.
Performs other duties and responsibilities assigned.
Knowledge and Skills
• Computer/IT knowledge including, ability work in Windows based system, MS Office (Excel, Word, Outlook, PowerPoint), SharePoint, Cyber.
• Strong written and verbal communication skills, ability to work effectively with families and youth with respect to services and supports.
• Ability to adapt to changing needs and respond to critical incidents.
• Ability to work independently as wells as lead Child/Family Team and coordinate services with system providers, partners, families and the public.
• Strong organizational skills and ability to plan and manage deadlines.
• Ability to understand and mediate between and among the demands of various systems.
• Knowledge of system partners e.g. Division of Developmental Disabilities, Department of Education, and Juvenile Justice Commission.
• Knowledge of mental health theories, methods and practices
• Knowledge of the local community regarding social, economic and other problems encountered by the population served.
Standards and Competencies:
• Adheres to HIPAA standards and maintains confidentiality.
• Complies with all agency policies, practices and procedures.
• Maintains a valid driver’s license in good standing. Has access to a vehicle available with automobile insurance for business use.
• Maintain availability for before and after-hours meetings as required.
• Participates in mandatory meetings and trainings
• Capable of and willing to conduct in-home visits/meeting and transport youth/families in a company vehicle.
• Maintains eligibility to work in NJ State DCF and Medicaid funded programs.
• Maintains high level of professionalism at all times.
• Participates in regular supervision and performance evaluation process.
• Maintains highest ethical standards.
• Maintains a strength-based approach, fosters collaborations.
Qualifications
Qualifications:
Bi-lingual skills English/Spanish required
*Master’s degree in social work, psychology or related discipline
*Related discipline will be considered with significant related experience
At least five years clinical experience in a child service system environment.
LCSW or LPC license preferred.
Previous supervisory experience preferred.
