Case Management Referral Specialist
Full time 40 hours per week
Location: North Providence
Specific Responsibilities:
Act as primary contact for the at Home Cost Share Program referrals requests and inquiries within the Senior Services and Adults with Disabilities Department. Receive intake referral forms from a variety of sources including but not limited to, The POINT, the consumer, family members, caregivers, community Partners such as, Senior Centers, Visiting Nurse Agencies, Home Health Agencies, on a continuous basis. Utilize a person- centered approach to evaluate the consumers and family needs, provide information and referral for alternate community needs.
Within required timeframes, conduct additional screening to verify the referral information provided is accurate, the types of services needed or requested, and to ascertain the applicant’s willingness to accept services. Explain the RI Office of Healthy Aging At Home Cost Share program eligibility rules, regulations m and application process to prospective applicants. Coordinate with clients, caregivers, or representatives in retrieving required documentation for program eligibility verification. Compile, record, and evaluate personal, medical, and financial data to verify completeness, accuracy, and to determine preliminary eligibility qualifications. Contact consumers, family members, caregivers by phone, in writing, and/or electronic communication to ensure they have completed required steps. Follow up on pending referrals. Communicate with prospective consumers, updates to appropriate referral sources, as needed. Discuss the pros and cons of applying for Medicaid, relative to an individual’s preferences, support system, resources, needs and any other factor the individual wants to address. Disseminate resource materials to consumers, caregivers, and community partners.
Maintain referral statistics for all programs within the department, input data management databases and complete data entry as required. Compile referral reports for Director or designee.
Qualifications:
- Minimum of High School Diploma or GED
- One year of relevant experience (e.g., providing information and referral or other type of assistance) working with the target population- Elders and Adults with Disabilities.
- Bi-Lingual with English and Spanish speaking abilities preferred, not required
- Pass state and national criminal background check
- Possess strong computer knowledge and be skilled in Microsoft Office products, and other computer system databases.

- Knowledge of available program funding and other resources

- Demonstrate the ability to work with people from various cultures and lifestyles in the helping process
- Demonstrate active and focused listening skills
